Dearest of patients,
Thank you for taking the time to update (or fill out for the first time) your paperwork for us.
Out of the utmost concern for the safety of our patients and our team; given the current epidemic, we will be requiring the following of our patients:
-You will be required to wear a mask and have your temperature taken upon entrance. If you do not have a clean mask to wear upon arrival, we will provide one for you.
-No gloves are permitted in the facility except by our doctors and healthcare team.
-Please adhere to social distancing requirements of 6 feet apart. We will permit no more than 10 people in our optical retail space at a time.
-All patients will be required to fill out check-in forms ahead of time and text pictures of their insurance cards (front and backside) to us to limit the amount of exposure in the office.
-Only the patient is allowed in the facility unless the patient is a minor or requires additional assistance by a caretaker or family member.
-Bring your medication list with you, if you have one. Better yet, text a picture of it to us.
-Bring all eyewear currently worn to your appointment.
-If you are a contact lens wearer, please take note of the brand of contact lenses you wear as well as the contact lens solution you use (we'll be quizzing you later!). Take pics and text them to us, if that's easier.
-If you or your child are considering contact lenses for the first time: Please be aware that there is a separate contact lens fitting fee that may or may not be covered by your insurance. This fitting, by law, has to be updated every 12 months in order to have an active contact lens Rx and be able to order contact lenses.
